Diversity is Beauty Project – Information, Compliance & Release Forms

Dear Model,

Thanks for working with Appeal Photography on the Diversity is Beauty Project!

Prior to the shoot we need for you to complete three forms online (see links below):

1. Model Release Form. This is a standard model release with the exception that it indicates that you will receive edited imagery from the shoot with rights to use it for your portfolio, social media, your web site, etc.
2. 2257 Compliance Form. This form is required as verification that you are over 18 years of age. It is required ONLY if you have never shot with Appeal Photography before. If you need to submit a 2257 form, you will also need to upload a copy of your photo ID, using the form on the right hand side of this page.
3. General Information Form. In this shoot we hope to share some information about you, including your ethnicity and, optionally, a few sentences about you and your perspectives on diversity. This will help to make the final imagery more personal and real, which will, in turn, foster a greater emotional impact.

All of the personal information you submit is stored securely on our server and never shared with third parties. For #3 above we will only post or include in other content information that you have chosen to share (i.e., ethnicity, background about yourself and perspectives on diversity).
