Aspring Models Program – Information, Compliance & Release Forms

Dear Model,

Thanks for being part of Appeal Photography’s “Aspiring Models Program!”

There are a number of forms we need you to complete prior to your participation in the program (forms 1-3 below) which need only be completed once. Form 4 is completed for each photo shoot.

All of the personal information you submit that you have not specifically given us permission to display or share is stored securely on our server and never shared with the public or third parties.

  • Aspiring Models Program Agreement

    The Aspiring Models Agreement establishes the basis of the relationship between the model and Appeal Photography LLC, including respective responsibilities, model preferences, etc. and is required by all participants in the Aspiring Models Program, co-signed by Appeal Photography LLC.

  • 2257 Compliance Form

    This form is required as verification that you are over 18 years of age. It is required ONLY if you have never shot as a model with Appeal Photography LLC before. You will also need to upload a copy of your photo ID(s), using the form on the right hand side of this page.

  • Aspiring Models Program - Model Information

    Please share a few sentences about yourself — your interests, background, aspirations, etc. to be used on your profile, in your portfolio, photo competitions, etc. Please only share information you are comfortable making public.

  • Aspiring Models Program - Model Release

    This model release must be completed for each photo shoot. This is a standard model release with the exception that it indicates that you will receive edited imagery from the shoot with rights to use it for your portfolio, social media, your web site, etc.
